What this form is.

  • A verification of the experience you gained under one supervisor, at one setting.
  • It carries totals, not weekly rows. The weekly rows stay on the 37A weekly log.
  • You usually need one per supervisor you worked under.

Who signs it

Your supervisor. This is the form where they state, in their own name and under their own license, that the experience happened.

Common mistakes.

Most are easy to catch while the work is fresh.

  • Totals that do not add up to the weekly logs behind them.
  • One form covering two supervisors or two settings.
  • Asking a supervisor to sign it after they have left the setting, with no record to show them.
