What this form is.

  • A running log of your supervised experience as a Florida registered intern, kept while you work rather than written at the end.
  • It is read in fourteen-day windows. Florida's rule is at least one hour of supervision every two weeks, so a window with client hours and no supervision stands out.
  • It sits alongside the core Florida numbers: at least 1,500 face-to-face psychotherapy hours and at least 100 supervision hours, spread over at least 100 weeks.

Who signs it

You and your qualified supervisor. Florida's log form says it may be signed electronically, under chapter 668, part I of the Florida Statutes.

Common mistakes.

Most are easy to catch while the work is fresh.

  • Counting hours from before your intern registration. Time before registration does not count in Florida.
  • A two-week window with client hours but no supervision. Catch it while you can still book a session.
  • Mixing individual and group supervision into one number. They are recorded separately.
  • Writing a client's name or initials anywhere on the log. It is an hours record, not a clinical one.
